It's not a bug, it's image's caption. Ideally, it should function as a proper caption.. but since many books don't provide one, the fallback is image's file name (what happens on screenshots). There's a reason why you can't hide it (at least for now).
A chapter must contain at least one paragraph of text for various reasons. For example, if chapter has only of image (and it's caption), hiding both would leave an empty chapter, which is not ideal. Completely hiding the chapter would be even worse, as it might have an important title.. There are also a few other things to consider.
I may change this behavior (one related feature is #130)
