Mixed Bundles

The bundled code spec seems to assume only a single type of code will be bundled. We have rates where DRGs and Revenue codes are part of a bundle, and other rates where Revenue Codes and CPT codes are part of a bundle.

These bundles have a price a price when any of listed DRGs are billed with any of listed Revenue Codes for first example. and any listed Revenue Codes bill with any CPT codes in the second example

The Billing Code Type, Version and Billing Code are mandatory fields in the in-network object and the bundled code object, the in-network object values should be allowed to be null if there is a bundled arrangement, so it should be conditional not mandatory.
