Metadata improvement

Not to forget to discuss once we get closer to the release:

• We should align predicate in concept_relationship metadata with the categories we use (broadMatch, narrowMatch, exactMatch)

• Reuse flag could be improved too (not quite sure what else is needed @dimshitc, but date would be nice)

We also need to add description of the process to annotate mappings.
I found several pairs where it seems that the annotation can be improved (although not sure):

2104139 maps to 4263085 - narrowMatch
2104016 maps to 4328656 - broadMatch
1572266 maps to 36717032 - narrowMatch (funny mapped from for this pair IS broadMatch but maps to is exactMatch). Also, here exact match exists in SNOMED (44784134).
