Problem

In both the app and the health summary, the dizziness score for patients is logged as a number between 0 and 5 which is confusing to patients. We think that it would make more sense if there could be the word label associated with what each number actually means. Below is the word label that we would want for each number (it is slightly different language than what is used in the survey). To clarify, the survey should stay the same, but if its possible we would like to have the words below associated with the numbers.

0 - None
1 - Minimal
2 - Mild
3 - Moderate
4 - Severe
5 - Very Severe

Solution

On the app, in the heart health page when dizziness is selected, it would be great if the number listed at the top of the graph could also include the word label associated with it next to it.
Ex: if a patient scored a 2 on Dizziness we would want the screen to say "2 - Mild"

On the health summary, instead of listing the number as the data for the patient, it would be great if we could have the words listed in the problem above instead. For this one, we just want the word to be listed, not the number associated with it.
