Capturing use of equipment #1078

For the development of the costing model, we will need to know what equipment is being used in the course of a model simulation. However, currently this information is not captured. For some HSI, there are comments saying that a piece of equipment is needed; for others it is probably not noted at all.

We need to:

  1. Decide a means of allowing the HSI to declare the equipment that is used in the course that HSI.
  2. Make those declarations for all the HSI where this is already obvious from the comments.
  3. Ask disease modellers to look at their modules and complete any other declarations needed.
  4. Integrate these declarations into the HSI catalogue & provide to Sakshi and Newton developing the costing module.

Point 1 (from PR description) done in the PR #1098.

I didn't find much in the comments about equipment. (Looked up 'todo' and 'equip'.) Only two TODOs from Andrew in the breast_cancer module, which I used to add some provisional equipment to be able to try running sims with equipment usage declaration and logging.equipment use declaration and logging.
