[Suggestion] Issue templates

I would like to suggest that some issue templates be made up in order to try to get reports organized and needed information provided at issue open.

I wouldn't mind time permitting to have a stab at doing some,

Here are some examples Ive done for an org/projects I develop.

Obviously these are organization wide so all projects trigger them, but it can exist in the specific project if so is desired.

What I would need to know, is more or less what types of information you most need to know and which types of templates you feel your organization would benefit from for your community.
