Consolidated master list

We have all the master lists separated by document type, since it's easier to build a table that way. But it would be useful to have everybody in one integrated table, with a title, type, and every other relevant column.

This does present some information presentation challenges, as enumerated in #45, but can be done with enough effort - and is worth doing!
