[bug] `kid`, `keyRef` and `alias` are all used to refer to keys and not interchangeably

This makes usage of the KeyManager plugin and development of other Key Management Systems cumbersome and prone to errors.
These different ways of referring to keys must be removed and a single one picked.

A fix for this will probably be a breaking change as it would require implementers of KMS to adapt.
