[v16] Wrong URL

Information about bug

It is best explained with picture, see the marks 1, 2, & 3 in the attached picture from /hr/roster/. First I got to that page by clicking on Roster from the menu within the HR app (Shift & Attendance).

2 issue to obverse:

1. clicking on Mark 1 leads to a wrong url path /app/hr
2. see mark 3, the favicon does not use the set favicon but default frappe favicon
