Bootstrap 4.0.0 stable version class names have changed, Timepicker no longer works #373

Input-group addon classes in Bootstrap 4.0.0 have changed which causes bootstrap-timepicker to no longer work.

JS and CSS need to be changed to reflect the new input-group-append and input-group-text class name changes in BS 4.0.0 stable release!
