Normal shuttles are really easy to depressurise, even by random pets who bump into the doors.
It should be reasonably easy to do this on purpose and even throw people outside, but should be more difficult to do by mistake.
We may be able to solve this on shuttles which don't have a real air lock (only a single set of doors) by placing a button on the wall nearby which bolts and unbolts the doors. The doors would close and bolt automatically as soon as the shuttle takes off (which could potentially crush someone, but that's their fault).
This is a little bit like the door release levers on airplanes, and would still allow you to open them reasonably quickly for nefarious reasons, but would not allow Paperwork to space himself.

* Partially fixes window smoothing
We were using alt icon states improperly, leading to weird smoothing
when a wall was directly below a window
Things still look wrong mind, but now they're at least uniformly wrong
Potentially may be accepted by upstreams.
