Do not change PR title when reworking on a branch

When a comment lands in an issue, or the ralph-review slash command is used on a PR, the ralp-loop usually changes the PR title to include just the last work. I.e. "feat: new feature" was renamed to "chore: use subtests", which is not accurate at all.

The reviewer must be aware of this, and update the title and description with the full context of the branch
