Improve migration flow with extend options

If i over looked it in the documentation apologies.

I think there is a need to improve the migration generation process to allow for alter table functionality via code changes in the model file.

Manually editing and adding migrations can introduce unnecessary risk and side effects, and is a bit dated with the ability to used extend functionality and have the query written with mikronORM
