While working on the kvalobs migrations I discovered that cfailed in both flags.kvdata and flags.old_databases seems to be a tag/string, and it's not just a simple index of the QC check that failed or the total number of failed checks.

Correct. It is actually a space-delimited list where the first string is the latest QC to have processed the data. Before manual intervention, it is usually just a string representing which QC1 or QC2 test raised the alarm. This can then be followed by HQC corrections later, and even manually triggered QC tests, which will add elements to the list.
