Here are a few ideas that have been previously discussed or off the top of my head. Please add to the list if you think of more.

artifacts outside the head
motion artifacts?
small fov
still contains nose and/or front of face
deface mask applied at wrong orientation
skull-stripped
low-res
part of nose and/or face wrapped around to other side
T2w and other non-T1w anatomicals
mri_deface and other non-pydeface defacing software
