This has no effect on the running of commands, except when typing the literal "yes" which doesn't show in the terminal. Executing the literal "yes" causes the terminal to output four times instead of two. I removed all .zXXX files and .zprezto/* including the .zshrc to see if it was zsh or zprezto. The issue went away using bare ZSH. Then I re-cloned zprezto and reinstalled and the issue reappeared. I've attached a picture showing what the issue looks like and a log from running zsh -x and that's the only command run.
