Add collections list to contact form when user chooses "Request access to another collection"

In the contact form, when the user chooses the option under "How can we help you?" that says "Request access to another collection," a list of options should appear. See below for how this looks in H2:

The selection(s) they make should also be sent in the email. I've just noticed that this is not currently happening in H2, but it should be.
