Add collections list to contact form when user chooses "Request access to another collection" #670

In the contact form, when the user chooses the option under "How can we help you?" that says "Request access to another collection," a list of options should appear. See below for how this looks in H2:

The selection(s) they make should also be sent in the email. I've just noticed that this is not currently happening in H2, but it should be.
