Set up Open Data Census on Heroku

Not having heard back from the Open Data Census team, I suspect that we're going to have to set up our own server. I see that CFA hosts their own census on Heroku, so I'm inclined to do the same.

I've got this running locally, thanks to guidance from @ondrae.

I have all of the states added, with their GNIS IDs. I have datasets added to the Google Sheet, but they're not showing up. I'm working on figuring out why that is.

I changed from GNIS IDs to USPS state abbreviations, since the IDs become the URL slugs. For Wyoming, a URL slug of 49 is a lot less useful than a slug of wy.
