This is a follow up to #739 and #738.

To be consistent with the proposed changes in the title and wording of the tip, I would change the way the example form is laid out, so the labels are sitting right above the form elements, instead of the left. I would leave the checkbox as is, but maybe align it with the rest of the stack by pushing it to the right a little.
