This makes a few cleanup changes that align things for the long term: - [x] remove use of `bytes` nullifiers - [x] move anchor into `TransactionParameters` - [x] move fee into `TransactionParameters` - [x] eliminate `Fee` type in favor of `Value` - [x] align `TransactionPlan` with `Transaction`
